Required Forms
If registering online, the required medical forms are included in the online registration process. Campers MUST have their own health insurance, and each camper must complete and return medical history and medical release forms to participate. WE CANNOT PERMIT THE CAMPER TO BE A PART OF CAMP UNLESS THESE FORMS ARE RETURNED.
